Registration

Registration deadlines and cost:

• Early registration is open from May 6 - June 8 -- $125.00
• Regular registration June 9 - June 22 -- $145.00

Refund Policy
$50 service for cancellations received prior to 6/16/09. 
After 6/16/09 no refund.

Online Registration Instructions

STEP ONE

registration is closed

1. Click on the “Register Now” link.
2. Complete the online registration and select method of payment. Cost for both teens and adults is $125 for early registration, $145 after June 8th.
3. Registration is not completed until you have clicked the “Finish My Registration” button. All payment is due no later than June 28th.

If you need to make a change to your registration, you must contact Tamara Kirk,  WSU Professional Education, at tlkirk@wsu.edu  or phone (509) 335-4248.

1. If you are paying for your registration with a check, please make out the check to “Washington State University” and include your name on the notation line on the check. Mail payment to:

      4-H Teen Conference
      PO Box 5222
      Pullman, WA 99164-5222

• If you need to cancel your registration, you must email Tamara Kirk at tlkirk@wsu.edu .  Cancellations received after May 31 will be refunded the registration fee paid, except for a $50 service fee.

 

STEP TWO

Any youth who arrives at the Conference without submitting these forms will be sent home at their own expense.

1. Print copies of all Registration forms, including
   • Form A Packet - Risk Assumption and *Image/Voice Consent
   • Form B Packet - *Health and Emergency Medical Release
   • Form C Packet - *Code of Conduct
   • Spanish version - Risk Assumption form
   • Spanish version - Parent Release form

   *Available only in English

2. Get the required signatures on these forms. You are responsible to make sure all of the signed forms in Packets A, B and C get to 4-H Teen Conference.
3. The responsible adult driving you to 4-H Teen Conference will check with you when you leave for Pullman (either on the bus or in a private vehicle) to make sure you have all three form packets. You will turn these in at 4-H Teen Conference when you get there.
4. Before you return your chaperone will give you Form A Packet back to have with you until you arrive home.
